Holidays Act Remediation Form

Former Kmart New Zealand Team Members can use the below form to request remediation for previous leave underpayment.

Step 1. Download form

Download and complete the Kmart Remediation Payment Claim form (PDF 232 KB).

Step 2. Scan documents

Collect and scan the relevant supporting documents:

  • IR330

  • Identify verification (of Team Member or executor/administrator) Please provide any one of the following documents:

    • Birth certificate,

    • Passport,

    • Certificate of citizenship,

    • Immigration New Zealand visa,

    • Drivers licence,

    • Firearms licence, or

    • HANZ 18+ card

  • Evidence of appointment as executor/administrator of the estate (if applicable)

Step 3. Email form/documents to Kmart

Scan and email the completed form and supporting documents to holidaysactnz@kmart.com.au

What happens next?

Kmart will process your request and send an email when completed.
