Download and complete the Kmart Remediation Payment Claim form (PDF 232 KB).
Collect and scan the relevant supporting documents:

Identify verification (of Team Member or executor/administrator) Please provide any one of the following documents:
Birth certificate,
Passport,
Certificate of citizenship,
Immigration New Zealand visa,
Drivers licence,
Firearms licence, or
HANZ 18+ card
Evidence of appointment as executor/administrator of the estate (if applicable)
Scan and email the completed form and supporting documents to holidaysactnz@kmart.com.au
Kmart will process your request and send an email when completed.
